You work closely with your Care Designer to create a premium experience and deliver on client care goals. </p><p><strong>Duties and Responsibilities </strong></p><ul><li>Creates Happier Aging experiences and client focused care in home and community settings</li><li>Delivers on our Brand Promise by owning the client’s experience, ensuring that you do whatever it takes to bring peace of mind to our clients and their families</li><li>Delivers comprehensive nursing care and assessments to clients according to your scope of practice </li><li>Engages and reports in open communication with your Care Designer when opportunities to elevate your client’s experience and care are identified </li><li>Responds to Care Services Center (CSC) team regarding scheduling inquiries and updates within the agreed upon timeline</li><li>Function within the limits of your own experience and knowledge, while practicing safely and competently </li><li>Is a leader in the community </li><li>Able to use applications via phone and email to ensure proper communication and documentation are completed for your client’s care</li></ul><p> </p><p><strong>Availability: </strong></p><ul><li>Flexibility is required</li><li>Casual / Part time / </li></ul><p><strong>Business Relationships: </strong></p><ul><li>Reports to the Care Designer  </li><li>Works closely with the Client Care Ambassador  </li></ul><p><strong>Key Metrics: </strong></p><ul><li>Attendance & Reliability </li><li>AMS - Attendance Monitoring System (AlayaCare)</li><li>NPS; Client Score of 10 </li></ul><p><strong>Requirements: </strong></p><ul><li>Two positive professional references</li><li>Current practicing registration as a Registered Nurse with CNO license</li><li>Minimum 1 year RN experience, home health care experience is considered an asset </li></ul><ul><li>Ability to travel independently to client visits</li><li>CPR and First Aid Certification</li><li>Exceptional written and verbal communication skills</li><li>We provide care 24/7, so visits may be scheduled early morning, afternoon, evening, or overnight to meet our clients’ needs.</li></ul><p><br><br></p></p></p>
